  • Assist client in selecting a plan

  • Client Name
  • DOB
  • Client Address
  • Insurance Name
  • ID # 
  • Insurance Phone Number
  • Subscriber Name and Relationship
  • Referred By (Treatment Center)
  • Copy of the Insurance Card if available
